currently, I use Media-tagger, which work nice with large amount of files aswell as its free

Generally the crap records get pushed to the back of your box and the ones you play often in the front...

Vinyls are never arranged, theres just one pile, last played are on top. CD's arent organized that well either, but usually i have similar CD's grouped together, usually the groups are Japanese Hardcore, Japanese Pop, UK Hardcore, Dutch Hardcore, EuroDance/trance, Punk, Finnish Pop and the rest are where ever i can fit them.
mp3's are organized using the folders iTunes creates when recording CD's:
Artist [folder] -> Album [Sub folder], for compilations its:
Compilations [folder] -> Compilation name [Sub folder]. Though after i started using the finnish iTunes, it didnt use the old compilation folder, but created a new one instead named Kokoelmat. So now all the newer ones are there. But i manage them with iTunes anyway, which allowes me to sort them however i want.
